YOUTH / YOUNG ADULT PEAK PERFORMANCE CHAPTERS

As is the case with all Upstream year-round programs, our Youth Peak Performance programming has a particular emphasis on helping young people achieve their highest athletic or performance goals. The team of subject matter experts for these members has deep experience in the peak performance field and a particular passion when it comes to optimizing medical, mental, dietary and movement practices for youth with very focused goals and objectives.

  • Your own personalized multi-disciplinary peak performance health team, guided by your lead medical coach
  • An unhurried intake and thorough physical exam with your lead medical coach, with a particular focus on peak performance indicators and injury prevention
  • Quarterly check-ins (3 subsequent to your initial intake) with your lead medical coach
  • An integrative plan developed by you and your medical lead outlining your best use of credits for follow up appointments and / or diagnostic procedures.
  • A 20 credit bank to use towards elective services as recommended by your medical lead, including options for additional appointments with your Nutrition, Mental Wellness or Functional Movement leads

  • A thorough and comprehensive intake with a qualified Registered Dietician or Nutritionist, experienced in peak performance nutrition
  • A 20 credit bank to use towards elective services as recommended by your medical lead, including options for additional appointments with your Nutrition, Mental Wellness or Functional Movement leads

  • A mental wellness and emotional resiliency assessment with a practitioner who is qualified, credentialed and experienced in the field of mental peak performance
  • A 20 credit bank to use towards elective services as recommended by your medical lead, including options for additional appointments with your Nutrition, Mental Wellness or Functional Movement leads

  • A thorough fitness and functional movement assessment by an experienced physiotherapist or kinesiologist.  Additionally, your functional movement coach will provide recommendations aligned with each individual’s specific performance goals.
  • A 20 credit bank to use towards elective services as recommended by your medical lead, including options for additional appointments with your Nutrition, Mental Wellness or Functional Movement leads

Membership

Upstream's Youth program has an annual membership fee of $2,295 (when bundled with at one or more Ascend or Summit program) or $2,795 (individual). For corporate pricing please call or email.

Base Camp Health Assessment

Our Base Camp Health Assessment includes 12 months of 24/7 access to doctors via our telemedicine service, in additional to an annual one-day multi-disciplinary assessment that includes a comprehensive medical exam and critical benchmarking along with expert evaluations and recommendations for optimizing functional movement, mental wellness and nutrition.

Ascend Health Solutions

Our year-round Ascend Health Solutions program includes 24/7 access to doctors via our telemedicine service. Ascend Health Solutions also provides for a comprehensive medical intake in addition to nutritional, functional movement and mental wellness assessments. You and your medical lead then create a personalized plan based on the results of your intake process and your personal health goals. You can apply your bank of 60 credits towards various services ranging from additional appointments with clinical experts to diagnostic procedures so that your health journey is focused on measurable and valuable outcomes!

Women's Perimenopause & Menopause Programs

Upstream’s Essentials program is designed to provide women with a foundational understanding of their health and to explore pharmacological options for consideration, including Hormone Replacement Therapy (HRT) among other interventions. Members also engage with nutritional, functional movement and mental wellness experts to ensure a holistic approach. Additionally, members receive access to Upstream’s telemedicine* service with access to healthcare professionals 24/7.

Men's Vitality Health Programs

Upstream’s Men’s Vitality Health program may be the program for you if you are experiencing any one of the following: potentially low testosterone levels, low energy, loss of muscle mass or ED (erectile dysfunction). Assess whether or not Testosterone Replacement Therapy (TRT) is right for you. And learn how to age well by better understanding the range of possible interventions and strategies from a team of multi-disciplinary healthcare experts.

Prediabetes / Diabetes Health Program

Nearly one in every 3 Canadians lives with prediabetes or diabetes. Diabetes contributes to 30% of strokes, 40% of heart attacks and 70% of all non-traumatic leg and foot amputations. Managing prediabetes and diabetes is possible and Upstream’s team of multi-disciplinary healthcare experts will help get you on track.

Iron Infusion Clinic

Upstream Health offers medically supervised iron infusions in a safe environment with virtually no wait times.

FEES

Membership fees for the medical components of Upstream programs are for preventative care. More specifically, these fees are for components of care that are not publicly insured. Any publicly insured services provided by Upstream will be billed to the provincial health insurance plan by Upstream or by its physicians. Subscription to an Upstream program is not a pre-requisite or in any way necessary to access insured services.
The fees for some Upstream services may be recoverable through a Member’s personal health and welfare plan. Upstream will issue receipts to Members’ for claim submission purposes at the request of the Member.
