Ascend


Our year-round Ascend program provides high level medical, mental wellness, functional movement and nutritional programming. You can also select up to four additional appointments with the coach of your choice (nutrition, mental wellness, functional movement). Additionally, you can use your bank of 10 credits towards discretionary services, based on the recommendations and guidance of your team of coaches. The Ascend program provides you with a team of coaches working together to create an integrative health strategy designed around your distinct profile and personal goals.

ASCEND PROGRAM CHAPTERS

  • Your own personalized multi-disciplinary health team, guided by your lead medical coach, who will meet with you as required throughout the year.
  • Through Upstream’s insurance partners, membership includes expedited access to CT and MRI scans where medically necessary.
  • Through Upstream’s insurance partners, membership includes expedited access to a network of specialists, (where medically necessary and when the public access takes more than 21 days), or assistance navigating fee-for-service options for discretionary or preventative matters.
  • An unhurried intake and thorough physical exam including:
    • A personalized medical intake and medical history assessment
    • At program start and at mid-year for benchmarking purposes, in depth and comprehensive lab work (blood and urine) indicating bio-markers for metabolic health, muscle health, fluid balance, cognitive health, fitness, oxygen transfer and liver function
    • A functional exam of heart, lungs, ears and eyes
    • A lab-based ECG test and cardiovascular risk assessment based on age and risk profile
    • Prescription and supplement review
  • An integrative plan developed by your 4 health coaches collaborating on a customized report for YOU.
  • A 10 credit bank to use towards discretionary services as recommended by your medical lead.
  • Critical Illness coverage will be provided for each member.

  • A thorough nutrition and dietary consultation with a qualified Registered Dietician.
  • Two subsequent appointments with your nutrition coach for further strategizing and adjustments.
  • Access to Upstream’s network of preferred partners for further services including meal planning, meal delivery service and more.

  • A mental wellness and emotional resiliency assessment with a qualified, credentialed and experienced practitioner.
  • Two subsequent appointments with your mental wellness coach to assess progress and adjust recommendations as required.
  • Referrals to Upstream’s network of preferred partners for further services including peak performance experts, wellness and therapeutic options and more.

  • A thorough fitness and functional movement assessment by an experienced physiotherapist or kinesiologist along with recommendations aligned for each individual’s personal goals, ranging from improved body composition to increased flexibility, improved endurance and strength and peak performance goals.
  • A demonstration session with your functional movement coach.
  • Two subsequent appointments with your functional movement coach to assess progress and to provide recommendations for adjustments to existing routines.
  • Access to Upstream’s network of Registered Massage Therapists, Physiotherapists and Chiropractors.

Base Camp

An annual one-day multi-disciplinary assessment that includes a comprehensive medical exam and critical benchmarking along with expert evaluations and recommendations for optimizing functional movement, mental wellness and nutrition.

Limited to 75 members, our year-round Ascend program provides high level medical, mental wellness, functional movement and nutritional programming, along with preferred rates for movement classes (yoga, pilates, stretch). You can also select up to four additional appointments with the coach of your choice (nutrition, mental wellness or functional movement). Your team of coaches work together to provide an integrative health strategy designed around your distinct profile and personal goals.

Summit

Limited to 50 members, our year-round Summit program provides high level medical, mental wellness, functional movement and nutritional programming. Additionally, the Summit program includes detailed genetic testing / analysis and a comprehensive gut health assessment. You can also select up to six additional appointments with the coach of your choice (nutrition, mental wellness or functional movement). Your team of coaches work together to provide an integrative health strategy designed around your distinct profile and personal goals.

$8,485/year each for family of 2+ | $8,995/year for individual

Youth / YA

Effective integrative health care that starts at an early age can have a significant impact on future health outcomes. Upstream’s Youth / Young Adult programming is designed for those aged 14 – 24 years of age and has two specific streams: the Youth / Young Adult Peak Performance program and the Youth / Young Adult General Integrative Health program.

FEES

Membership fees for the medical components of Upstream programs are for preventative care. More specifically, these fees are for components of care that are not publicly insured. Any publicly insured services provided by Upstream will be billed to the provincial health insurance plan by Upstream or by its physicians. Subscription to an Upstream program is not a pre-requisite or in any way necessary to access insured services.
The fees for some Upstream services may be recoverable through a Member’s personal health and welfare plan. Upstream will issue receipts to Members’ for claim submission purposes at the request of the Member.
