Our year-round Ascend Health Solutions program includes 24/7 access to doctors via our telemedicine* service. Ascend Health Solutions also provides for a comprehensive medical intake in addition to nutritional, functional movement and mental wellness assessments. You and your medical lead then create a personalized plan based on the results of your intake process and your personal health goals. You can apply your bank of 60 credits towards various services ranging from additional appointments with clinical experts to diagnostic procedures so that your health journey is focused on measurable and valuable outcomes!

  • Telemedicine*: 24/7 access to doctors and nurse practitioners through GreenShield Health; primary healthcare at your fingertips.
  • Your own personalized multi-disciplinary health team, guided by your medical lead, who will meet with you monthly for the first 6 months of your membership.
  • An unhurried intake and thorough physical exam including:
    • A personalized medical intake and medical history assessment
    • An in-depth and comprehensive blood panel indicating bio-markers for metabolic health, muscle health, fluid balance, cognitive health, fitness, oxygen transfer and liver function
    • A functional exam of heart, lungs, ears and eyes
    • A lab-based ECG test and cardiovascular risk assessment dependent on age and risk profile
    • Prescription and supplement review
  • An integrative plan developed by you and your medical lead outlining your best use of credits for follow up appointments and / or diagnostic procedures.
  • A 60 credit bank to use towards elective services (Services Menu) as recommended by your medical lead.

  • A thorough nutrition and dietary consultation with a qualified Registered Dietician or Nutritionist.
  • Additional appointments with your Nutrition lead, as desired, using your credit bank (Services Menu).
  • Access to Upstream’s network of preferred partners for further services including meal planning, meal delivery service and more.

  • A mental wellness and emotional resiliency assessment with a qualified, credentialed, and experienced practitioner.
  • Additional appointments with you mental wellness lead, as desired, using your credit bank (Services Menu).
  • Referrals to Upstream’s network of preferred partners for further services including peak performance experts, wellness and therapeutic options and more.

  • A thorough fitness and functional movement assessment by an experienced physiotherapist or kinesiologist along with recommendations aligned for each individual’s personal goals, ranging from improved body composition to increased flexibility, improved endurance and strength and peak performance goals.
  • Additional appointments with you functional movement lead, as desired, using your credit bank (Services Menu).
  • Access to Upstream’s network of Registered Massage Therapists, Physiotherapists and Chiropractors.

Membership

Upstream’s Ascend Health Solutions program can be paid up front with a fee of $5,595 / member for Family memberships (2+) or $5,895 for an Individual membership. If 12 monthly installments are preferred the following fees apply: Family memberships (2+): $395/month and $995 down. Individual memberships: $425/month and $995 down.

Upstream’s Youth / Young Adult programming is designed for those aged 14 – 24 years of age and has two specific streams: the Youth / Young Adult Peak Performance program and the Youth / Young Adult General Integrative Health program.

Women's Perimenopause & Menopause Programs

Upstream’s Essentials program is designed to provide women with a foundational understanding of their health and to explore pharmacological options for consideration, including Hormone Replacement Therapy (HRT) among other interventions. Members also engage with nutritional, functional movement and mental wellness experts to ensure a holistic approach. Additionally, members receive access to Upstream’s telemedicine* service with access to healthcare professionals 24/7.

Upstream’s Men’s Vitality Health program may be the program for you if you are experiencing any one of the following: potentially low testosterone levels, low energy, loss of muscle mass or ED (erectile dysfunction). FEES

Membership fees for the medical components of Upstream programs are for preventative care. More specifically, these fees are for components of care that are not publicly insured. Any publicly insured services provided by Upstream will be billed to the provincial health insurance plan by Upstream or by its physicians. Subscription to an Upstream program is not a pre-requisite or in any way necessary to access insured services.
The fees for some Upstream services may be recoverable through a Member’s personal health and welfare plan. Upstream will issue receipts to Members for claim submission purposes at the request of the Member.
